Accessing someone else’s information (Proxy access)
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Passport signing
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
- Medical record requests
- Certain travel immunisation
- Housing letters
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff or Practice Manager will be happy to advise you. For letters and certificates appointments are sometimes not required. Please ask reception for details and note that requests should be put in writing.
