To Register with The Practice

If you wish to register click and open the 2 forms and print them out.  

When you have completed all of the details bring BOTH forms to reception. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Please note the supplementary questions in GMS1 are not mandatory.

When you register you will also be asked to sign a medical declaration. This is to ensure that Larkshall Medical Centre can provide you with the most appropriate service for your medical requirements.
