Informal List Closure

Larkshall Medical Centre will not be accepting any new patients and have an Informal list closure to new patients

GP Practices have a duty of care to provide quality, accessible and safe care to patients. We also have an obligation to review and take appropriate measures if we deem our workload would compromise patient safety or the care we provide.

Unfortunately, we feel unable to accept any new requests to register at this time as we are facing unprecedented demand and shifts in work from secondary care that is overwhelming our systems

Please be assured that we have not taken this decision lightly. Nor does this decision in anyway relate to applicant race, gender, social class, age, religion, sexual orientation, appearance, disability or medical conditions.

You should bring your NHS number and the details of your previous GP with you and see the receptionists who will help you complete the documentation.  A registration forms and health questionnaire can be downloaded. Please print these forms and complete them with your details and hand it to reception. Please come to register between 9am and 5pm only. Thank you

Your records can be retrieved electronically and securely by NHS systems from your old GP.

As soon as you register with this practice we arrange your complete health check to ensure that you are free from any disease. Please bring in any current medication you are taking.

We register patients mainly from the E4 postcode of London and some patients living on the Essex border of Chingford.
